Sport Physical Due Dates

  • Fall Sports Physical forms due by July 15, 2024

  • Physicals valid 365 from the start date of first day of practice

  • A Health History Update is to be done for each sport season, it is done on Genesis (Genesis will be open for fall forms on July 1, 2024)

  • A covid clearance is to be completed if you had it 90 days prior to the start of your sport season. 


ONLY HARD COPIES WILL BE ACCEPTED. No emailed forms will be accepted. Please drop them off to the main office or mail them to the school attention: Nurse’s Office. Thank You.
