Welcome Letter

Dear Incoming Class of 2030 Parents/Guardians:

Please review the information below and complete all required forms before the start of school.

ALL INCOMING 9TH GRADE STUDENTS

REQUIRED: LINK TO FORMS

● Physical exam completed within 365 days of the first day of school.

● Updated immunization records.

● Physicals must be signed and stamped by your child's healthcare provider.

● Either a Universal Health Form or Pre-Participation Physical Evaluation Form (all 5 pages)

Submit all paperwork by email, mail, or drop off at the Main Office during summer office hours.

Students who do not submit the required physical and immunization records could be excluded from school as per NJ State Law NJAC 6A:16-2.2.

STUDENTS WITH MEDICAL CONDITIONS

If your child has any of the following, additional forms are required: LINK TO FORMS

● Allergies

● Asthma

● Diabetes

● Seizures

● Prescription medication needed during school hours

All forms must be renewed every school year.

The OTC Medication Form (Tylenol, Advil, Tums) can be completed in the Genesis Parent Portal.

All medications brought to school must:

● Be in the original pharmacy-labeled container.

● Be delivered to and administered by the School Nurse.

● MUST have a Prescription Medication Form filled out by their Doctor

Students may NOT carry or self-administer medication unless approved by the School Nurse.

Registration Opens: July 1, 2026

ALL ATHLETIC PAPERWORK IS DUE BY JULY 20, 2026

Please note: Athletic clearances will be updated periodically throughout the summer. Most clearances will be processed after July 20, 2026.
